NREMT Exam Overview
The NREMT cognitive exam is a computer-based test that ranges from 70 to 120 questions. The time limit is 2 hours. Ten of these questions are unscored pilot questions, used to test new material.
The exam covers the following content areas:
- Airway, Respiration & Ventilation (18–22%)
- Cardiology & Resuscitation (20–24%)
- Trauma (14–18%)
- Medical/Obstetrics/Gynecology (27–31%)
- EMS Operations (10–14%)
Most questions focus on adult patients (85%) with the rest covering pediatric care (15%).
You’ll need to apply both knowledge and clinical judgment. The exam is somewhat about knowing symptoms, but it’s majorly about making the right decisions under pressure.
What Is the Psychomotor Exam?
Besides the cognitive exam, there’s also a psychomotor skills test. This hands-on exam assesses your ability to perform critical EMT procedures in real-time scenarios.
Note: This exam is not administered by the NREMT. It’s usually handled by your training program or state EMS agency.
Skills you’ll be tested on include:

Frequently Asked Questions
Am I a certified provider when I pass the NREMT?
Not yet. Passing the NREMT cognitive exam is one step toward certification.
To become a certified EMT, you also need to pass a psychomotor (skills) exam and meet any additional state-specific requirements. Once all steps are complete, you’ll be officially certified and eligible for state licensure.
Is the NREMT a government agency?
No. The NREMT (National Registry of Emergency Medical Technicians) is a non-profit, independent organization. It sets national standards for EMS certification but is not part of the U.S. government. However, many states use NREMT exams as part of their EMT licensing process.
What is the hardest part of the NREMT exam?
The adaptive format and scenario-based questions.
Many test-takers find it difficult because questions are based on real-life situations, not just facts. The Computer Adaptive Testing (CAT) format also makes it harder to predict how you’re doing since the difficulty adjusts with your answers.

Is 120 questions on NREMT good?
It depends. Getting 120 questions means the exam needed more data to assess your ability.
It doesn’t mean you passed or failed. Some pass in 70 questions, others at 120. The number of questions alone doesn’t determine your result.
What is the lowest score on the NREMT?
The lowest possible score on the NREMT is 100, and the highest is 1500. To pass, you need a score of at least 950.
But this test doesn’t work like school exams where you get a percentage grade. You’re not trying to get a certain number of questions right. Instead, the NREMT exam measures how well you can handle real-life EMT situations. The system checks if your answers show that you’re ready to be an entry-level EMT.
So even if you feel like you got a lot wrong, you can still pass, as long as your answers show that you meet the standard.
What percent is correct to pass NREMT?
There’s no set percentage like 70% or 80%. The exam is adaptive and doesn’t grade based on how many questions you get right.
Instead, and as mentioned earlier, it looks at your ability level over time. If you meet the required level of competency, you pass, even if you got many questions wrong along the way.
How many chances do you get on the NREMT?
You get 3 attempts per application.
If you fail 3 times, you’ll need to complete a state-approved refresher course before you can retake the exam. After that, you can apply for 3 more attempts.
Does NREMT tell you what you got wrong?
No. If you fail, the NREMT gives you a performance report showing which content areas you struggled with (like airway, trauma, or EMS operations). It doesn’t show which specific questions you missed.
